Abstract

The integration of artificial intelligence (AI) is increasingly recognized as a promising way to improve equity in surgical care. This paper explores the impact of AI in surgery across five main areas: key applications of AI, strategies to reduce bias, ethical AI implementation, effects on patient outcomes and access, and future prospects for equitable AI in surgical care. The first section highlights the potential of AI in surgery, focusing on how these technologies can enhance decision-making, increase precision, and optimize patient care. AI-driven algorithms have the capacity to improve surgical outcomes, personalize treatment plans, and streamline procedures, benefiting patients from diverse backgrounds.
The second part discusses efforts to minimize bias and promote equity within AI-enabled surgical care. It underscores the importance of using diverse and representative datasets, ensuring algorithmic transparency, and applying fairness metrics. Addressing bias in AI is key to reducing healthcare disparities and ensuring that surgical care is fair and equitable for all patients. The third section addresses the ethical considerations of implementing AI in surgical settings. It stresses the need for transparent governance, informed consent, privacy protections, accountability, and continuous ethical evaluation. Transparent governance systems are critical to maintaining accountability, resolving ethical concerns, and mitigating biases.
The fourth section examines the impact of AI on patient outcomes and access to surgical services. AI technologies have the potential to improve decision-making, enhance surgical results, and streamline patient care. However, it also emphasizes the need to address issues like bias, privacy, and ethics to ensure responsible and fair implementation of AI. The final section looks at the future of AI in surgical care, highlighting areas such as the development of robust data infrastructure, advances in machine learning, explainable AI, AI-driven surgical automation, and addressing health disparities. These advancements offer significant opportunities to improve patient outcomes and enhance equitable access to surgical care. By incorporating AI into surgical practice, healthcare systems can not only improve surgical outcomes but also reduce inequalities and guarantee more equitable access to care.
To ensure the responsible and fair use of AI in surgery, future research and collaboration are essential. Responsible deployment must focus on mitigating bias, adhering to ethical guidelines, and staying attuned to the evolving nature of AI. With careful implementation, AI can greatly benefit both equity and patient-centered care in surgical settings.
