A Policy-Based Model for Strengthening Antimicrobial Stewardship Programs in Emerging Economies

Abstract

Antimicrobial resistance (AMR) poses a significant and growing threat to global health, with emerging economies particularly vulnerable due to high infectious disease burdens, unregulated antimicrobial use, and weak health system infrastructure. Antimicrobial Stewardship Programs (ASPs) are recognized as a critical intervention to optimize the use of antimicrobials, improve patient outcomes, and mitigate the development of resistance. However, the effectiveness of ASPs in emerging economies is frequently limited by fragmented policy frameworks, insufficient regulatory enforcement, inadequate human and financial resources, and low adherence to clinical guidelines. This abstract presents a policy-based model designed to strengthen ASP implementation by integrating governance, evidence-informed protocols, and multi-stakeholder coordination. The model emphasizes three core dimensions; regulatory alignment and policy coherence to ensure national antimicrobial guidelines, licensing requirements, and hospital accreditation standards support stewardship objectives; capacity development, including targeted training for prescribers, pharmacists, and laboratory personnel, coupled with standardized monitoring and evaluation tools; and operational integration through digital tools for real-time prescription tracking, resistance surveillance, and performance feedback. The model further incorporates multi-level governance mechanisms to facilitate accountability across ministries of health, hospital management, and professional associations, ensuring that ASP policies are effectively translated into practice. Implementation pathways include phased pilots in sentinel hospitals, continuous policy evaluation, and stakeholder engagement to adapt interventions to local epidemiology and resource constraints. Anticipated outcomes include reduced inappropriate antimicrobial prescribing, improved clinical outcomes, enhanced laboratory diagnostics integration, and strengthened surveillance of resistance patterns. This policy-driven approach provides a scalable framework that aligns stewardship initiatives with national health priorities, addresses systemic barriers, and promotes sustainable improvements in antimicrobial use. By combining regulatory oversight, capacity-building, and digital integration, the model offers a practical roadmap for emerging economies to combat AMR while maintaining equitable access to essential medicines.
